Wait for coverage confirmation
After coverage is confirmed, your Field Reimbursement Manager (FRM) or the RYZUP team will provide your office with a prepopulated LUMRYZ Prescription Form (patient demographic information only) and directions on where to fax.
Submit the LUMRYZ Prescription
